Answering a query - Window Example A

Queries that are assigned to you appear underneath the purple bar in the window. Queries assigned to other providers appear underneath a gray bar. To open a query, click on it. You can open queries assigned to other providers.

Answering a query sends information back to the clinical documentation improvement (CDI) reviewer. The system administrator defines the number of queries shown per patient each time you open the window. Therefore, you may answer requests and then see more requests for the same patient the next time you open Fluency Direct.

The name of the query template that was used is displayed in the header of the window. The top panel displays Patient Demographic data, including the Pation Name, Gender, Age, Date of Birth, Admit Date, and Location. You can click the MRN and Account number to copy it to your clipboard so you can paste it into another location.

The indicator in the upper right corner of the window shows whether the query is only assigned to the receiving clinician or if it is also assigned to other people.

The text of the query is displayed on the left-hand side of the screen. It is important to read the entire text first to ensure the correct response. The date and time the query was sent, as well as the name of the person who sent the query, is displayed below the query text.

If your site uses the chart launch functionality available in your EMR, clicking on the patient name opens the chart review for the same patient in the EMR.

To answer a query
  1. In the CDI Queries window, click the record for which you want to answer queries.

  2. Choose the desired option under Response Option or Other options. Depending on the option you choose, you may have be able to leave a comment, your own diagnosis, decide to document in the EHR, or click Save.

  3. Close the window or click the arrow ( < ) to return to the list of queries.

Once you have made your response selection, the query closes and you will return to the CDI Queries window. If all queries for all patients are complete, the CDI Queries window will close.

If applicable, a Query Response document will be created and sent to your EHR. Whether or not your choice results in a Query Response Document is configured by an administrator. Please consult your 360 Encompass System documentation for additional information.

Note

If you have multiple requests for the same patient, none of the responses are sent until you answer all of the requests, return to the patient list, change patients, or exit.

In addition to the listed Response Options, which will vary based on query type, you can choose one of the following under Other Options:

  • Dismiss - Not applicable/Not valid. Select if you disagree with the question posed. It does not apply to the patient's circumstances. If you select this option, you can include comments (optional). Click Save when done.

  • Dismiss- Clinically Unable to determine/Unknown.

  • Refer to clinical documentation reviewer. This option appears only if the request was created by a reviewer. It enables you to create a message back to the reviewer and completes the request (it does not reappear on your list.)

  • Assign to another provider. If your facility has enabled this ability, you can assign the query to another provider, if you think they are the appropriate person to review and answer the query. Enter a provider name in the search field. Only providers set up to use 360 Encompass appear as a match.

When you click Save, the query is immediately removed from your list and does not appear in the Recently Completed list.

To return to the list of queries, click arrow in the the demographics bar at the top of the panel. The next time you open the CDI Queries window, the query will not be listed.
